NICU Transition to Home Program Navigator

The primary goal of this position is to facilitate the safe transition to home for premature and medically complex infants and coordinate effective follow-up post-NICU discharge, including the coordinated outpatient management of home oxygen and home gavage feeds

About our Division:

The UMass Memorial Health Children's Medical Center (CMC) is Central Massachusetts' primary provider of general and specialty pediatric services. The CMC houses the region's only Pediatric ICU, Level III NICU, Pediatric Level 1 Trauma center, hospitalist-led pediatric inpatient services, a pediatric sedation service, a designated pediatric emergency department, and over 20 pediatric specialty services with a focus on high quality care, education, and academic excellence. We are very focused on our anchor mission for the children and families in this community.

Caregiver Requirements:

* Conducts comprehensive health assessments, demonstrating a high level of autonomy and expert skill in diagnosing and treating morbidities of prematurity * Formulates clinical decisions to manage acute and chronic illnesses and promote wellness * Demonstrates leadership and initiative to ensure follow-through to address patient-specific needs * Utilizes and integrates clinical-based medicine to facilitate protocol development and integration. Participates in research and education to best care for medically complex infants * Effectively engages in collegial relationships with nursing peers, physicians, professionals, and others who strive to enhance the healthcare for premature and medically complex infants * Provides health care in accordance with standards of practice as delineated by state practice guidelines and national board certification organizations
