Job Description: Cornell Scott-Hill Health is in search of a Licensed Practical Nurse for openings in various locations!The Licensed Practical Nurse is responsible for giving clinical support to providers and providing clinical and supervisory assistance to nursing staff. The LPN will advocate for patients seeking and receiving care and provide support to the Medical Assistants. Bi-lingual Spanish speaking highly desirable.Responsibilities Include But Are Not Limited ToPerforms screening tests and order routine lab work with medical standing ordersCompletes specific treatment or nursing procedures as ordered by the provider Assists in providing outreach services to patients who miss their appointments Responsible for placing orders and managing all imaging studies including Ultrasound, CT scan, MRI and Gynecology imagingPerforms triage functions of the patients and routes the patient to the appropriate clinical serviceAdministers medications, immunizations and treatments as per written order of the provider. Adheres to the "Medication Reconciliation Act"Responsible for completing telephone triage Obtains EKG's and blood pressure checks to be reviewed by a ProviderProvides supportive nursing counseling related to common acute medical problems and to primary preventative care for patientsContributes to in-service education and helps identify areas where neededAttends mandatory monthly Nursing supervisory meetings and occasional weekend trainings as requiredPerforms all other duties as requiredRequirementsLPN Graduate from an accredited school. Must be registered as an LPN in the State of Connecticut. Connecticut LPN licensure required. Experience in Internal Medicine, Surgical Specialties, Long Term Care or Rehabilitation required. Experience performing EKG's and triaging patients also required. Experience performing phlebotomy is essential. CPR Certification required. Bi-lingual Spanish speaking highly desirable. Excellent Patient Management skills. Must be strongly motivated and able to accept supervision. Computer skills are essential with the ability to learn Centricity, and Yale EPIC. Ability to provide phone triage along with knowledge of referral process, medication refill procedures and Hollister charting are also required. Knowledge of CLIA waived lab testing, Urine Dipsticks and Gucometers are necessary. Excellent leadership qualities and knowledge of management skills are necessary.
