Job Description
- Engage in morning rounds and follow up care in clinic
- Serve patients in the outpatient setting for post sepsis care and to help reduce readmissions
- Improve the management and recovery from sepsis
- Full time, M-F, no call schedule creating a work life balance
- Underpinning knowledge of the Sepsis and associated risk factors or willingness to earn
- Collaborate and work directly with our critical care department
- Collegiate and well-established providers comprised of 30+ physicians and advanced practice providers
- Utilize EPIC EMR

Qualifications
- Master of Science degree with a completion of an accredited Nurse Practitioner or Physician Assistant Program
- Board Certification as a Nurse Practitioner or Physician Assistant
- Proven experience working with and sepsis patients as an NP/PA preferred, new graduates are welcome to apply.
- Current state licensure as a Nurse Practitioner and Registered Nurse.
