Nurse Practitioner (MSN) - Geriatric Medicine

The Mass General Hospital Division of Palliative Care & Geriatric Medicine is seeking a Per Diem Nurse Practitioner or Physician Assistant to join the Geriatric Medicine Service as a member of its outpatient and inpatient care team in support of our innovative and exciting clinical program expansion.
The Geriatric Medicine Service delivers high-quality, comprehensive, and compassionate care via team-based collaborative model to patients who are 65 and older. We serve a diverse, medically complex, and socially vulnerable population. The outpatient service provides primary care and consultative services. The inpatient service focuses on medical co-management with the Ortho-trauma service for patients with various traumatic fractures and assist with pre-op optimization, reducing postoperative medical complications and reducing overall length of stay. The inpatient team also offers geriatric consultation for older adults to assist in managing or reducing the risk of geriatric syndromes including but not limited to delirium, polypharmacy, frailty. We utilize the Age Friendly Health System 5Ms Care Model.
A successful Advanced Practitioner will be clinically outstanding, collaborative, and compassionate with excellent communication skills. This role includes day-to-day management and direct care of patients through co-management with the attending physician/other APPs, including diagnostic and therapeutic interventions, development of appropriate plans of care and ongoing evaluation. The APP should have in-depth knowledge and clinical expertise in managing the care of older adults.
The Advanced Practitioner role may include outpatient and inpatient rotation. In the outpatient setting, responsibilities include collaborating with attending physicians and other interprofessional team members to manage patient care. We provide both primary care and consultative/co-management care models in clinic for a wide range of issues including routine follow up for chronic disease and health promotion, urgent care visits for new or exacerbation of chronic health problems, wellness visits, post-discharge care after hospitalization for acute illness, serious illness and what matters most discussions and care-giver stress management.
In the inpatient setting, the Advanced Practitioner attends on service with physicians and trainees. Each provider has a panel of patients to manage with a combination of ortho trauma patients and general geriatric consultative patients.
In addition, the Advanced Practitioner will have responsibilities for patient, family, and staff education; clinical consultation; admission and discharge coordination; clinical research; and clinical program development.

* Current Massachusetts licensure as Advanced Practice Nurse (NP) or Physician Assistant (PA) * National certification as Advanced Practice Nurse (NP) or Physician Assistant (PA) * Massachusetts Controlled Substances Registration * DEA Controlled Substances Registration * Certification in Basic Life Support
Education:
* Graduate from an accredited Nurse Practitioner or Physician Assistant program * Master's degree preferred * Advanced training in the care of older adults desirable
Experience:
* A minimum of two years of inpatient experience is strongly preferred * Knowledge of Geriatric syndromes, 5M's Geriatric Care Model, serious illness conversations
