Position Summary:

Under the direction of the Program Director, the Case Manager will provide Person Centered, strength-based case management to Adults with Severe Mental Illness (SMI.) The Case Manager works in partnership with the other members of the Client's Mini Team, which includes Clinicians, Nursing, Peer Support and Prescriber Services.

Essential Responsibilities:

  • Develop and maintain rapport with individuals
  • Provide telephone outreach and home/community visits if needed.
  • Provide "hands-on" case management support to clients, including providing short-term transportation to support engagement with providers and social services.
  • Link individuals to resources and valuable information to improve health and wellness.
  • Help advocate for individuals to improve their access to care.
  • Complete required written documentation per program expectations.
  • Perform case management tasks utilizing life experiences to help and aid understanding to both consumer population and to staff.
  • Provide peer support services such as information, training and advocacy in order to assist individuals regain control over their own lives and over their own recovery process
  • Facilitate mutual support and self-help groups
  • Demonstrate an ability to develop respectful interpersonal relationships with consumers while maintaining appropriate boundaries.
  • Help guide consumers in developing WRAP and or other wellness plans
  • Provide supervision for consumers while on unit.
  • Assist consumers with researching placement and benefit options.
  • Assist consumers with applying for and securing housing and benefits.
  • Assist with and contribute to multi-disciplinary planning.
  • Respects diverse views and approaches, demonstrates Standards of Respect, and contributes to creating and maintaining an environment of professionalism, tolerance, civility and acceptance toward all employees, patients, and visitors.
  • Grade C02 Case Manager I: Associates degree in Human Services or HSD or HISET.
  • Grade C04: Case Manager II: Requires a Bachelor's degree; preferably in the field of psychology or human services.
  • Experience working with addictions and mental health issues.
  • Effective communication and listening skills.
  • Comfortable working independently and within a group.
  • Flexible yet organized.
  • Bilingual (Spanish/English or Vietnamese/English) a plus.
  • Must have a current valid US-issued driver's license and ability to provide registered, inspected, and insured automobile for work related purposes including transporting clients in your own vehicle.
  • HSD or equivalent is Required for this role. Associates degree or bachelor's degree in human service field is desired.
  • Ability to hire, train, supervise and evaluate program staff and to ensure that required competencies are maintained.
  • Ability to oversee clinical operations including behavioral and rehabilitative treatment planning for mental health and substance abuse clients.
  • Demonstrate effective communication with consumers, staff, peers, and other professionals.
  • Operate program in an efficient and effective manner ensuring compliance with BSAS, DPH, HUD, DMH and MAP regulatory requirements.

  • Previous/current participation in MH system as recipient of service and a willingness to convey this experience as relevant, and must be certified or be eligible for the Mass Peer Specialist training and be certified within 6 months of being hired.
