Job Overview:
Shriners Children's Ohio has an opportunity for a part-time Social Worker for our Care Management department. Join a team of professionals committed to improving children's lives. Hours: The hours for this part-time position is 24 hours per week and the preferred schedule is three 8 hour days per week.
Responsibilities:
- The Social Worker is responsible for providing assessments, planning, interventions and follow-up for patients and family members at Shriners Children's across the continuum of care.
- Through this function, by joining the team, you'll have the opportunity to make an impact on our patient's experience.
- You'll help to provide continuity of care for positive outcomes in our family centered environment.

Qualifications:
Required:
- Masters in Social Work
- Ohio Social Work license
- Case Management Certification (CMC, ACM or ANCC) required or the ability to attain certification within 2 years of hire or within 2 years of eligibility
- Three years of social work experience
- Experience conducting and documenting psychosocial assessments and interventions to meet physical, functional, social, emotional, educational needs of the patient/caregivers
- Experience in a healthcare setting with community and national resource navigation and coordination, payment methodologies, mitigating psychosocial barriers to patient/family adherence
Preferred:
- 5-7 years of social work experience
- Previous social worker experience in a pediatric setting
- Bilingual in English/Spanish
