Peer Recovery Specialist

Full Time | Day Shift | Monday-Friday | 8am-5pm

Experience based pay rate up to $21/hour

Lutheran Social Services of Central Ohio is currently seeking a Peer Recovery Specialist for LSS Health Center (formerly Health Center at Faith Mission) in Columbus, Ohio. What will I do as a Peer Recovery Specialist with Lutheran Social Services?
  • Provides outreach, engagement, and support for patients who experience overdose and other substance use disorders in assigned Emergency Departments.
  • Works closely with LSS Health Center staff and Urban Health to identify, enroll, coordinate, and monitor participants in the program.
  • Links patients to treatment, support, and resources in the community as needed.
  • Work with patients to plan and monitor care:
    • Assess unmet health and social needs.
    • Develop care pathways.
    • Monitor progress toward care pathway accomplishments and facilitate changes as needed.
    • Follow up with patients, staff, and community resources to coordinate care.
  • Follows patients in the community to make sure pathways to recovery and other needed services and supports are in place.
  • Participates in community meetings to promote recovery.
  • Attends recovery support meetings with patients when appropriate.
  • Develops and maintains professional relationships with external community resources.
  • Consistently communicates pertinent and/or timely information to LSS supervisor, community partners, and other team members.
  • Documents activities in electronic health record and other formats as needed, including progress notes, referrals and appointments made, and outcomes of patient referrals.
  • Attends and provides input at monthly team meetings to improve quality and coordination.
  • Provides feedback for the improvement of the Peer Support Specialist Program.
REQUIREMENTS for a Peer Recovery Specialist with Lutheran Social Services:
  • High-school diploma or GED required; Associate degree preferred.
  • Peer Support certification or ability to complete education/training and acquire certification within first 6 months of employment required; certification must be maintained.
  • CPR/First Aid/AED certification or ability to complete training within first 6 months of employment required; certification must be maintained.
  • Lived experience of addiction and recovery from substance use required.
  • Some social service experience preferred, either professional or volunteer, especially experience working with people in recovery.
  • Knowledge of recovery services and supports available in the community.
  • Knowledge of general community services and resources.
  • Ability to work effectively with people from various cultures, backgrounds, religions, and socioeconomic groups.
  • Strong customer service background and ability to work with diverse populations including but not limited to homelessness, behavioral health, substance abuse/addiction, chronic physical illness.
  • Competent computer skills and ability to use business software, electronic health record, and internet applications.
  • Valid Ohio driver's license with access to reliable insured transportation and ability to meet LSS insurance standards required.
