Currently recruiting a Licensed Practical Nurse (LPN) - Patient Centered Medical Home (PCMH) in Fort Belvoir, Virginia to provide care to Active Duty heroes, their families, and retirees at Fort Belvoir Community Hospital. Services will be required M - F, between the hours of 0630 -2030. Services will be required on Federal holidays.
DUTIES OF THE LICENSED PRACTICAL NURSE (LPN) - PATIENT CENTERED MEDICAL HOME (PCMH):
- The Contract Licensed Practical Nurse shall perform the following procedures, as required, and in strict compliance with standing orders and protocols set forth by the MTF to include, but will not necessarily be limited to the following:
- Obtain vital signs, procure height and weight, wound irrigations, and suture removal, obtain and prepare specimens for laboratory testing, perform EKG/ECG, perform quality checks and assist with calling patients to remind them of appointments or results as directed by RN or provider.
- Assist provider with treatment and diagnostic procedures, preparing equipment and the area. Position patient for procedure, observe patient for physical or emotional reactions, provide reassurance during procedure. Assist with BCLS. Maintain readiness of crash cart.
- Maintain record of treatment and patient log sheets. May initiate arrangements for follow-up care or assist with patient check-in.
- Instructs patient self-care in accordance with provider instructions.
- Completes additional tasks as assigned by the clinic chief nurse or his/her designated representative.
- Demonstrate competence in starting/performing intravenous (IV) access and phlebotomy/blood draw for specimen collection or present certificate of attendance and or certificate of completing IV and phlebotomy class within 3 months of employment.
- Obtains initial health data in relation to the patient s psychosocial and physiological needs through observing and interviewing techniques.
- Perform on sight screening of patients with a wide range of medical problems.
- Administers Health Risk Appraisal questionnaire.
- Performs quality control checks and operator maintenance for all equipment used in the clinic.
- Provides educational services to patients and their families for the promotion of health and prevention and/or detection of disease, i.E. Prenatal care, self-breast examination, and blood pressure screening.
- Assists in identifying patient needs and implements plan of care.
- Documents cars given and patient s response in the electronic medical record.
- Recognizes signs and symptoms of impending emergency situations, assesses patient care needs, and institutes emergency nursing measures to include CPR.
- Prepares patients for physical examination and diagnostic tests such as, but not limited to draping and positioning patient for chest, abdomen, rectal and vaginal examinations. Prepares patient and treatment area for special procedures such as biopsies and I&D of cysts and boils.
- Carries out prescribed nursing and physician orders and document patient medical record.
- Performs a range of diagnostic support duties which includes but is not limited to obtaining throat cultures and initiating laboratory slips, isolating suspected contagious cases, and giving complete instructions on the proper method of collecting a specimen such as clean catch urine and sputum specimen. Explains and provides instruction sheets on the non-fasting and fasting blood work done in the laboratory.
- Performs electrocardiograms in the absence of the technician. Changes dressings, performs ear irrigation s, and tests patients urine and stool specimens.
- Administers prescribed medications and observes for desired action, dosage, and adverse reactions to the medication.
- Assists in patient/family education regarding prescribed medications/therapies/diagnostic procedures that must be accomplished.
- Communicates pertinent patient information and data to the team leader, charge nurse, or physician as appropriate.
- Possesses knowledge of actions and usual dosages of most commonly used emergency drugs and their location.
- Assesses patient/family knowledge level, implement plan of instructions and record patient/family demonstrated level of understanding.
- Assures content and correctness of a prepared report and sign document to validate its content.
- Participates in monitoring quality of care according to professional standards and established procedures.
- Responds to telephone inquiries regarding clinic services and/or follow-up ancillary reports.
- Carries our physician, FNP and PED written and verbal orders. Verbal orders are to be limited to emergencies only.
- Assists clerical personnel in administrative activities as needed to facilitate efficient clinic operations.
- Completes additional tasks as assigned by the clinic chief nurse or his/her designated representative.
- Utilizes the Composite Health Care System (CHCS) and AHLTA (electronic medical record). The Government is responsible for training the contractor in the use of CHCS. Also uses ADS and other automated data systems instituted by the US Army.
- The contract Licensed Practical Nurse performing services under this contract shall have sufficient skills to perform tasks that may include, but are not necessarily limited to the following:
- Complete all required paperwork Utilize paraprofessionals as appropriate Perform documentation and patient care IA- TJC guidelines.
- The Contractor HCP shall assume additional administrative or clinical duties as requested or needed to ensure sustainment of normal clinic operations and ensure compliance with TJC standards, to include, but not limited to, managing daily schedules, reviewing and dispositioning consults, monitoring appointment utilization and patient access to care, conducting mandatory clinical training, monitoring clinic supplies and equipment, ensuring enforcement of clinic SOPs and standards.
- Quality Improvement shall include the following responsibilities:
- Uphold established patient care and practice standards.
- Review and submit reports as required.
- May be assigned to other clinics in the FBCH footprint on a temporary basis as needed or for additional on the job training.
- Participate in all mandatory training.
- Education: Certificate/Degree/Diploma from an accredited community college, junior college, college, university or vocational nursing program approved by the appropriate State agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The National League for Nursing Accrediting Commission (NLNAC) or the Commission on Collegiate Nursing Education(CCNE).
- Experience: Have a minimum of one year of nursing experience as, an LPN or RN, within the past three years in a hospital setting or LPN's with military, medic, corpsman, NA/MA experience.
- Current BLS from the American Heart Association or American Red Cross
- Current, unrestricted license from any of the 50 states and U.S. Territories
- Must be a U.S. Citizen (for access to Government computer systems)
