Greater Bergen Community Action, Inc. is looking for full-time Community Health Workers/Resource Navigators to join the team at The Unity Health Partnership.

The Unity Health Partnership is a bold and simple idea-to bring mission-aligned, high-quality service providers who are committed to health access and equity, regardless of insurance status, under one roof. The Unity Health Partnership will house a customer-choice food pantry operated by Community Foodbank of New Jersey, mental health services provided by CBH Care, primary medical services provided by Bergen Volunteer Medical Initiative, public health offices, and Community Health Workers/Resource Navigators provided by Greater Bergen Community Action.

Our "no wrong door" approach and shared commitment to assessing clients' Social Determinants of Health challenges will ensure that all clients are connected to the services they need within Unity Health and other providers. Located in Garfield, Unity Health Partnership will be convenient to residents of the 7 nearby Bergen County communities with some of the highest rates of health disparities in NJ.

Unity Health is an innovative model because of its services and the collaboration that created it. It will be a national model for addressing Social Determinants of Health challenges for the most vulnerable.

The Community Health Worker/Resource Navigator will work with clients regarding their social determinants of health needs by conducting intake, assessment, and referrals to GBCA and partner services. This role will be the primary first contact for people seeking services at Unity Health and will be responsible for following up on referrals and assisting clients in getting the services they need.

Responsibilities:

  • Responsible for knowing all agency programs and qualifications for services.
  • Maintain familiarity with community resources and add to an active list of resources; establish methods for inter-agency referrals
  • Conduct outreach and disseminate information about the program to various community groups to ensure adequate client awareness of existing services.
  • Act as a liaison with other Greater Bergen Community Action (GBCA) divisions and programs to facilitate client access to the full range of agency services and partner agencies.
  • Maintain client records and program performance statistics and prepare compliance reports as the funding agency requires.
  • Follow up on referrals and keep case notes on each referral made.
  • Is responsible for entering data in various GBCA databases.
  • Perform monthly or quarterly basis reporting.
  • Represent the organization professionally and respect client confidentiality.
  • Performs other relevant duties as assigned.
Qualifications:
  • Certified Community Health Worker (CHW) with a high school diploma or equivalent with prior experience.
  • Preferred: College degree in social/human services or related discipline and CHW certification.
  • Must be able to maintain a high level of confidentiality and professionalism.
  • Good communication skills - Interpersonal, verbal, and written; must possess English/Spanish fluency.
  • Strong organization and time management skills.
  • A demonstrated passion for working with income-eligible communities and communities of color to advance equality.
  • Experience working with individuals from various cultural and socio-economic backgrounds.
  • Good computer skills - Microsoft Applications
  • Must be able to take oral and written directions, multi-task, and pivot priorities.
  • Must be an adaptable and cooperative team player. Collaborates with team members to ensure that all departmental needs are met.
  • Valid NJ driver's license and use of own vehicle for self-transportation; local travel may be required.
