POSITION SUMMARY : The PHP Manager is responsible for the overall functioning of the partial hospitalization program. Responsibilities include the development and implementation of a high-quality treatment program, the overall management of the partial program, supervision of the assigned clinical staff, and participation in the community relations program. The PHP Manager plans, implements, and oversees programs to meet the social and emotional needs of the patients and participates as a member of the multidisciplinary team and the unit community relations team. The PHP Manager will fulfill the role of supervisor for the Program.

Essential Core Functions:

Oversee other therapists, providers, nurses, and all other staff in the program are trained and able to provide quality of care withing practice standards and program requirements. Sees that psychosocial assessments, Plan of Action, and Plan of Action updates are completed within established timeframe. Develops and implements program plans and schedules. Sees that discharge planning in collaboration with the patient is completed timely. Demonstrates understanding of individualized, behavioral and measurable treatment goals based upon information on psychosocial assessment and psychiatric Sees that group therapy sessions and individual and/or family therapy sessions are completed with best practice and skill. Coordinate with psychiatrist and providers to ensure quality continuation of care and schedule weekly medication management Oversees insurance benefit information, authorization for treatment and perform ongoing utilization reviews are conducted within Hospital procedures and protocols. Responds appropriately to patient's requests and supervisory expectations. Provides comprehensive education, staff training, instructions, and/or advocacy information designed to assist the patient and significant others. Provides referring physicians and/or psychotherapists with weekly updates and team meetings. Prepare and assist with audits and program presentations when Establishes strategic social services program goals by gathering pertinent business, financial, service and operations information. Defines the philosophy, goals and objectives of social services processes and procedures under the direction of Hospital administration, Horizon's clinical and operations staff, and in conjunction with the Medical Director Works with divisional and national support staff to ensure that the client hospital realizes maximum value from Horizon services. Understands client hospital's expectations of Horizon and works to meet these expectations. Plans and evaluates program activities and outcomes, communicating the plans and progress to program stakeholders. Responsible for managing services provided to patients and their families. Provides leadership to subordinate social workers and therapists. Develops and implements social services training for the program. Works in partnership with the Program Director, community-based providers, Horizon leadership, client hospital administrative staff and clinical staff to develop and implement programs that promote behavioral health programs within the area/community. Represents the program and Horizon at hospital, client and community functions. At all times, conducts oneself ethically in accordance with all relevant standards and regulations. Maintains compliance with state and federal confidentiality laws as well as upholding Horizon's confidentiality policies and procedures. Maintains Knowledge of and demonstrates respect for patient rights. Coordinates work hours with the Program Director. Assesses program and patient progress, communicating results and trends to the Program Director and treatment team. Submits timely and accurate CQI+ outcome data timely. Attends and/or directs program and Hospital meetings as directed. Actively participates in department processes as requested Ensures facility is safe and reports deficiencies to the Program Director Maintains professional interactions with peers, senior management, and staff through effective written and verbal communications. Assists with ambulating and lifting patients as needed. Maintains all required state/regulatory, hospital and company competencies and training expectations. Qualifications

Education, Certification/Licensure, and/or Experience

A master's degree from an accredited college with a major in Social Work, Psychology, Sociology or similar field Must have a valid state license in Social Work or other similar fields required by the state such as LCSW, LPC or LMFT or state equivalent. Qualified as a state approved clinical supervisor. Minimum three years' experience in a similar position and/or industry. Is knowledgeable of Hospital and departmental policies and procedures, objectives, quality improvement program, safety, environmental and infection control standards. Basic knowledge of Hospital and Horizon senior staff and organizational structure. Thorough knowledge of the principles of management, including program planning, staffing, and budgeting. Ability to instruct, direct, and evaluate employees.
