BASIC FUNCTIONS: The Medical Records Technician is responsible for maintaining and completing medical records in electronic and paper formats; compiling, evaluating and processing all records for accuracy and completeness of patient's condition, treatment, and health outcome. The ideal candidate will provide excellent customer service and maintain good working relationships within the department and with other departments. The Technician must be timely with requests for information which come from a variety of sources including patients, clinical departments, Patient Financial Services, other healthcare facilities and legal entities. Compiles, reviews and files documentation of patient's condition, treatment, and health outcome. Assigns chart deficiencies in accordance with hospital policies and procedures Resolves medical record discrepancies by collecting and analyzing information. Maintains the master patient index within the organization's enterprise computer system to ensure accurate patient records Interacts with Patient Access, Patient Financial Services and physician office staff for information verification. Processes all requests for patient health information by the patient and for treatment, payment and operations purposes as allowed by healthcare regulations. Responsible for maintaining patient confidence and protecting hospital operations by keeping information confidential; following release-of-information protocols, policies and procedures, and legal requirements. Learns organizational policies and procedures as well as regulatory requirements and rely on instructions and pre-established guidelines to perform the functions of the position. Prepares statistical reports by collection and summarizing medical care and census information, such as types of diseases treated, surgery performed, and use of hospital beds. Collaborates with departments handling patient records and will be accurate in scanning & indexing documents to appropriate patient charts. Performs a variety of tasks and other duties as assigned. QUALIFICATIONS High school diploma or equivalent required. Health information management secondary education course work preferred. 1 year of relevant experience preferred. AHIMA or AAPC certification preferred; may be accepted in lieu of some years of experience.
