ASSIGNMENT DETAILS - Dates of Coverage: 6-12 months approx. Reason for Coverage: LOA and departure Practice Setting: Hospital Based-all inpt-OR ICU/Rounding of patients in ER and floors Schedule (Shifts and Hours): M-F 6:45AM-5PM Call Schedule: nights and weekends- TBD- approx. 7-8 calls per month Call Ratio: 3 providers taking call Average Number of Cases Per Day: 2 cases per day Trauma Level/Designation: Level One Annual ER Volume: 85,000 per year Number of Operating Rooms: 3 Documentation System/EMR: EPIC Description of Ancillary Staff: Full complement Adult and Geriatric Certification Requirement: BLS and ACLS Procedures Required: Descending Aortic Aneurysms, CABG, Off-Pump CABG, Arterial Revascularization -Using Internal Mammary Arteries, Radial Artery and Gastroepiploic Artery, Redo Coronary Revascularization, Tube Thoracostomy, Pericardial Window, Thoracic aortic aneurysms, Heart Valve Repair/Replacement Surgery, CoreValve System, Mitral Valve Repair SUBMISSION REQUIREMENTS - Board Certified-

Required BLS and ACLS-

Required CT license-

Highly preferred-
