Position Summary:
We are seeking Clinical Social Worker I/II's (LMHC) to work within our School-Based Health Division at Urban Health Plan. Our school health division spans across 12 locations, providing public school students with primary care, dental, and mental health services. As the LMHC within our school setting, you will provide support to students by way of behavioral health screenings, crisis intervention, and individual psychotherapy.
This position will be based at our program located at Fannie Lou Hamer Middle School and PS 66 in The Bronx.
Minimum Qualifications:
Minimum $72,000 per year; Maximum $80,000 per year. Contingent on previous experience working as an LMHC.
Work Environment Type, Location, and Schedule:
Onsite, Bronx, NY, Monday to Friday 8:00 am to 4:30 pm.

Job Requirements

Minimum Qualifications:
- Master's Degree in Social Work from Accredited Institution is required.
- NYS LMHC credential is required.
- 1-2 years of experience providing counseling
- Ability to thrive in a fast-paced environment as a member of an interdisciplinary team.
- Ability to identify high-risk psychosocial factors in patients and families that impact health status.
- Provides bio-psychosocial assessments and clinical diagnostic assessments.
- Conducts individual psychotherapy and counseling services to children within public schools.
- Maintains skills and knowledge in clinical theory and practice to help patients overcome dependencies, adjust to life, and make changes.
- Provides counseling to family members to assist with understanding and supporting patients.
- Engages in crisis intervention when necessary and refers patients or family to community resources.
- Provides consultation to medical providers. Collaborates with physicians to develop and implement treatment and intervention programs.
- Participates in clinical team meetings.
