Description

This LPN position will float between multiple sites / programs in Redmond and will need to have open availability. This position is eligible for night / weekend shift differentials.

JOB SUMMARY: The Nurse I or II works in the WMS (Detox) setting as a part of a multidisciplinary team to provide medical detoxification and stabilization services for detox and residential clients in compliance with all BestCare policies and procedures, best nursing care practices, and all local, state, and federal regulations. Responsibilities include provision of medication management, taking patient vitals, maintenance of pharmacy/medical supplies, coordination of care with primary care providers for our mental health clientele, and advocacy for excellent patient care to achieve the best patient outcomes.

ESSENTIAL FUNCTIONS:
  1. Works closely as part of a multi-disciplinary team to provide excellent patient care with improved outcomes for clients;
  2. Takes vital signs, such as blood pressure, temperature and CWAA scores;
  3. Identifies patient care requirements by establishing personal rapport with potential and actual patients and other persons in a position to understand care requirements;
  4. Establishes a compassionate environment by providing emotional, psychological, and support patients, family, and friends;
  5. Gives injections of medications; immunizations as prescribed by Program and Medical Director,
  6. Promotes patients' independence by establishing patient care goals; teaching patients, family and friends to understand condition, medications, and self-care skills; answering questions about drug and alcohol addiction;
  7. Assures quality of care by adhering to therapeutic standards; measuring health outcomes against patient care goals and standards; making or recommending necessary adjustments; following detox and nursing division's philosophies and standards of care set by the state board of nursing, state nurse practice act, and OAR's for detox programs;
  8. Resolves patient problems and needs by utilizing multidisciplinary team strategies;
  9. Ensures a safe and clean working environment by complying with procedures, rules, and regulations;
  10. Protects patients and employees by adhering to infection-control policies and procedures, medication administration and storage procedures, and controlled substance regulations;
  11. Documents client progress in electronic health record (EHR) system to maintain a pertinent record of client activity according to established methods and procedures;
  12. Participates in daily staff organizational meetings and treatment planning review meetings;
  13. Takes a lead role or participates in providing medication administration and medical services;
  14. Other related duties as assigned.

QUALIFICATIONS:

EDUCATION AND/OR EXPERIENCE REQUIREMENTS:
  • Current Practical Nurse (LPN) diploma for Nurse I or Associates RN degree for Nurse II
  • Experience working with mental illness and/or substance abuse disorders
  • For individuals recovering from the disease of addiction, must have maintained continuous sobriety for the previous two years at the time of hire
LICENSES AND CERTIFICATIONS:
  • Current Oregon License as Practical Nurse (LPN) for Nurse I or RN for Nurse II
  • Register Nurse (RN) license for Nurse II, or Licensed Practical Nurse (LPN) license for Nurse I
  • Must maintain a valid Oregon Driver License or ability to obtain one upon hire, and be insurable under the organization's auto liability coverage policy (minimum 21 years of age and with no Type A violations in the past 3 years, or three (3) or more Type B violations)
PREFERRED:
  • Bilingual in English/Spanish a plus
REQUIRED COMPETENCIES: Must have demonstrated competency or ability to attain competency for each of the following within a reasonable period:
  • Strong client engagement skills
  • High level of ability and comfort providing extensive outreach and community-based nursing care in a multitude of settings
  • Proven success in collaborating with co-workers and a multi-disciplinary team to provide and advocate for excellent patient care
  • Proficiency in MS Office 365 (Word, Excel, Outlook), EHR systems, databases, virtual meeting platforms, internet, and ability to learn new or updated software
  • Strong interpersonal and customer service skills, and demonstrated compassion towards individuals with mental illness and substance abuse disorders
  • Strong communication skills (oral and written)
  • Strong organizational skills and attention to detail, accuracy, and follow-through
  • Excellent time management skills with a proven ability to meet deadlines

    • Ability to maintain strict confidence as required by HIPAA, 42 CFR, and Oregon Statutes
    • Ability to build and maintain positive, collaborative relationships

TOOLS AND EQUIPMENT USED: Standard office equipment including computer/laptop, copy/fax/scanning machine, printer, telephone, or multi-line phone; Medical equipment including blood pressure cuff (digital and manual), pulse oximeter, digital thermometer, glucometers, venipuncture equipment, AED, stethoscope, gait belt, and oxygen tanks and tubing, etc.

  • Must be able to perform repetitive tasks/motions, have good manual dexterity
  • Must be able to lift/carry up to 20 lbs. frequently and push/pull 21-75 pounds occasionally
