Avante at Ormond Beach is seeking a Licensed Practical Nurseto join our team of long-term care professionals. This is a vital role in a facility with a positive community reputation, a history of successful outcomes, and an experienced cohesive team. This is a 144-bed skilled nursing facility, with an In-house therapy department.

We are currently looking for 7p-7a and 3-11 full time shfts.

Description:

The LPN primary purpose is to provide direct nursing care to the residents and to supervise the day-to-day nursing activities performed by Nursing Personnel in accordance with current federal, state and local standards governing the facility, and as may be directed by the Director of Nursing Services, Assistant Director of Nursing Services and Administrator, to ensure that the highest degree of quality care is maintained at all times.

Responsibilities for the lpn are:
  • Supervise, through routine rounding, nursing assistants and others involved in direct care of a specified group of residents;
  • Perform ongoing assessment of residents, make periodic reviews, monitor residents' treatment and medications and make independent decisions regarding nursing care to ensure residents are receiving proper care and alert supervisor to significant changes;
  • Consult with physicians in providing care, treatment, diagnostic and therapeutic services, alert physicians (per facility policy) to resident accidents, incidents and changes in residents condition, lab work, etc. and obtain orders as indicated;
  • Transcribe physician orders (per facility policy) and provide follow through (e.g., call pharmacy for new medication, x-ray, etc.);
  • Participate in the orientation of new residents and family members; inform personnel of new admissions, ensure that rooms are ready, greet new admissions and escort to rooms;
  • Admit, transfer and discharge residents as necessary
  • Provide written and oral reports to the Director of Nursing concerning status of residents, problem areas, etc.;
  • Prepare and administer medications in accordance with state guidelines, as ordered by the physician and observe for possible adverse reactions;
  • Review medication cards for completeness, accuracy, adherence and notify physicians of stop orders prior to the last dosage being administered;
  • Verify resident identifies to ensure prescribed medications are administered to the proper residents;
  • Ensure that narcotic records are accurate and notify the Director of Nursing of all drug and narcotic discrepancies noted on the shift;
  • Ensure proper disposal of drugs and narcotics;
  • Order prescribed medications, supplies and equipment as Necessary
  • Sign, date and perform all routine charting and record keeping in accordance with accordance with established policies and procedures and in accordance with State and Federal Guidelines;
  • Complete and chart all accidents and incidents involving residents and submit to the Director of Nursing as required;
  • Encourage physicians to review treatment plans and to record and sign orders, progress notes, etc.;
  • Chart nurses' notes in an informative and descriptive manner;
Qualifications for the lpn:
  • Must be aLicensed Practical Nurse in good standing and meet all applicable federal and
  • state licensure requirements.
  • Must be 18 years of age
  • Completion of secondary education or otherwise show ability to read and write in
  • a legible and understandable manner.
  • Must speak and understand English
  • Previous experience in a supervisory capacity
  • Must be knowledgeable of nursing and medical practices and procedures and
  • laws, regulations and guidelines governing long-term care.
  • Must maintain current CPR certification.
  • Positive attitude toward the elderly.
