Job Summary: We are seeking a highly skilled and compassionate Telehealth RN Case Manager who is bilingual in Russian and English to join our dynamic healthcare team. The ideal candidate will be responsible for coordinating and managing patient care remotely, utilizing telehealth platforms to provide efficient and effective healthcare services. As a Telehealth RN Case Manager, you will work closely with patients, healthcare providers, and other stakeholders to ensure continuity of care, promote patient education, and facilitate positive health outcomes. This role requires strong clinical knowledge, excellent communication skills, and the ability to leverage technology to deliver exceptional patient care remotely. Responsibilities: Conduct initial patient assessments and develop individualized care plans based on clinical findings and patient needs. Utilize telehealth platforms to monitor patient progress, provide health education, and coordinate necessary interventions. Collaborate with healthcare providers, including physicians, specialists, and allied health professionals, to ensure comprehensive care delivery. Assess and address patient concerns, escalating issues as needed to ensure timely resolution. Educate patients and caregivers on health management strategies, medications, and self-care techniques. Document all patient interactions, assessments, and care plans accurately and timely within electronic health records (EHR) systems.
