Job Description: Ensure the provision of cost-effective pharmacotherapy contributing to optimal patient outcomes, provide experiential education for present and future health care practitioners, and support drug-related research.Responsibilities:\n\nInterpret and evaluate physician orders in order to prepare, calculate, and dispense appropriate medication dosage forms for inpatient and outpatient treatment in accordance with the Practicing State's Pharmacy Practice Act, Federal Drug Laws, and other regulatory organizations.\nReview, maintain, and screen patient medication records and profiles for allergies, drug interactions and therapeutic duplications while entering orders and/or verifying medications.\nOrganize daily operational duties and assist in the supervision of pharmacy technicians, pharmacy aides, student interns and other ancillary personnel involved in providing pharmaceutical care.\nAdhere to all regulatory requirements, professional standards of practice, and hospital quality management policies and procedures in providing pharmaceutical care.\nCommunicate with physicians and nurses to ensure prompt clarification of orders that are unclear, illegible, or incomplete.\nProvide drug information and/or educational programs to healthcare professionals and pharmacy students.\nUtilize all available technologies in the provision of pharmaceutical care (i.e., computer systems and databases, Acu-Dose, Robot-Rx, pneumatic tube system, etc).\nFill prescription orders in accordance with State and Federal regulations, utilizing technological resources and equipment.\nPrepare and dispense intravenous admixture solutions, as required, using aseptic technique in an accepted sterile product preparation area.\nIdentify and report suspected adverse drug reactions accurately and in a timely manner.\nSustain the formulary by minimizing non-formulary procurements, utilizing therapeutic interchange protocols, and promoting rational drug therapy selection.\n\nB.S. degree in Pharmacy or Doctor of Pharmacy (Pharm D.) from an accredited school of pharmacy required.\n 3-6 months hospital pharmacy experience is preferred.\nPrevious experience or familiarity with investigational drug control procedures and protocol design is preferred.Licensure, Certifications, and Clearances:\nPharmacist\nAct 34\n\n UPMC is an Equal Opportunity Employer/Disability/Veteran
