LVN Service Coordinator

POSITION SUMMARY/RESPONSIBIILITIES
The Service Coordinator-Licensed Vocational Nurse (SC-LVN) collaborates with medical, behavioral, and social support staff within the Service Coordination team to provide a holistic evaluation of STAR Kids Members’ individual dynamics, needs and preferences while providing education and health-related information to the Member, the Member’s Legal Authorized Representative (LAR), and the Member’s Support Network. Coordinated STAR Kids members’ covered and uncovered services, including both acute and long term services and support (LTSS), while meeting the Member's physical, behavioral, functional, and psychosocial needs. Complies with University Health System’s and CFHP’s policies, procedures and protocols for establishing and maintaining good working relationships with co-workers, Health System employees, patients and guests.
EDUCATION/EXPERIENCE
A minimum of three years of clinical nursing experience is preferred. At least two years of experience in a managed care environment or working with people with disabilities and vulnerable populations who have chronic or complex conditions is preferred. Data entry experience required. Call center experience preferred.
LICENSURE
Current licensure in the State of Texas as an LVN Nurse is required.
