Performance Quality Improvement (PQI) Coordinator

Kaleidoscope Inc, a non-profit, child welfare organization located in Chicago, Illinois is looking to fill the position of PQI Coordinator to assist with the day-to-day performance and quality improvement data management activities for the Enhanced Clinical Community Support Program.

General Description

Under the supervision of the Director of Clinical Community Programs, the PQI Coordinator s responsible for assisting with the day-to-day activities of the performance and quality improvement system for the Enhanced Clinical Community Support and Intensive Placement Stabilization teams.

Essential Duties and Responsibilities:

  • Uses valid and reliable data collection methods to measure outputs, outcomes, and progress toward performance targets.
  • Prepares data reports from various quality improvement activities.
  • Assists in preparation for program monitoring/grant compliance visits in accordance with funder requirements.
  • Performs data entry for ECCS and IPS programs and reviews for trends and/or opportunities for improvement.
  • Organizes and creates ECCS youth client files and is responsible for filing paper submission to files.
  • Responsible for managing the IPS Database and data submission.
  • Analyzes CANS 2.0 data and identifies trends for quality improvement at time of discharge for youth in the IPS and ECCS Programs.
  • Analyzes CANS 2.0 data for youth currently enrolled in the ECCS Program every 180 days.
  • Analyzes Treatment Plans outputs for youth in the IPS and ECCS Program.
  • Identifies new data collection points based on gathered information.
  • Manages the Apricot Data System for the ECCS and IPS Programs.
  • Collaborate with the PQI Manager to ensure activities related to the Agency's quality improvement activities are effectively executed and documented as it pertains to the ECCS and IPS Programs.
  • Fosters an agency culture of continual improvement through proactive engagement of staff at all levels in the agency's performance and quality improvement system (PQI).
  • Participates in the implementation of the agency's PQI Plan.
  • Participates in the agency's PQI committee and collaborates with the PQI Manager, Director of HR and Operations, and Chief Program Officer as needed for committee support.
  • Manages the ECCS and IPS File Review process.
  • Facilitates the monthly ECCS and IPS PQI Meetings.
  • Creates the Quarterly IPS and ECCS Performance Reports
  • In collaboration with the PQI Manager, create the Annual Performance Reports.
  • Assists in the surveying of ECCS and IPS stakeholders to determine satisfaction with services.
  • Maintains a strong knowledge of DCFS and Agency policies and procedures.
  • Demonstrates the ability to utilize DCFS SACWIS and Performance Dashboard systems.
EDUCATION REQUIREMENTS/GENERAL QUALIFICATIONS
  • Minimum Degree Required: Bachelor's degree in social work or social service-related degree.
  • Minimum Years of Experience: Experience with continuous quality improvement, research, data entry and analysis preferred, but not required.
  • Information Technology Skills Required: Microsoft Office Proficiency and Client Database Software.
  • Other Requirements: Experience working with the Illinois Department of Children and Family Services or similar agency: Excellent coalition building skills with an ability to work effectively with a variety of internal and external stakeholders; Excellent interpersonal, oral and written communication, time management, and organizational skills.
