Provide comprehensive primary care services to patients across the lifespan. Build and maintain long-term relationships with patients to ensure continuity of care. Diagnose and treat acute and chronic illnesses, manage preventive care, and promote overall wellness. Collaborate with physicians and other healthcare providers to deliver coordinated, team-based care. Ensure adherence to evidence-based practices and standards of care. Educate patients and families about treatment plans, medications, and healthy lifestyle choices.
Candidate requirements
Board Certification as a Nurse Practitioner (NP). Active Nurse Practitioner license in New York State (NYS). Graduate of an accredited Nurse Practitioner or Physician Assistant program. Proven experience in primary care or family medicine settings. Strong interpersonal skills, with a commitment to kindness, care, and patient engagement.
