Position Overview:
LHH is currently seeking experienced Case Managers for our client in LA. At this organization, you will play a critical role in guiding clients through complex housing issues and situations by assessing their needs, developing tailored action plans, and coordinating with various service providers. You will be responsible for ensuring that clients receive the appropriate resources and support to achieve their goals and improve their quality of life.
Hiring method: Contract to Hire
Location: Los Angeles, CA
Hours: Multiple shifts available (day/mid shift/overnights)

Pay Details: $24.00 to $25.00 per hour

Key Responsibilities:
- Client Assessment: Conduct thorough assessments of clients' needs, strengths, and challenges to develop comprehensive care plans.
- Care Planning: Develop, implement, and regularly update individualized care plans that align with clients' goals and needs.
- Resource Coordination: Connect clients with appropriate community resources, services, and support systems, including healthcare, financial assistance, and social services.
- Case Management: Monitor clients' progress, adjust care plans as needed, and provide ongoing support to address any emerging issues or changes in circumstances.
- Advocacy: Advocate on behalf of clients to ensure they receive necessary services and benefits. Work to remove barriers to care and address any concerns or complaints.
- Documentation: Maintain accurate and confidential case records, including assessment notes, care plans, progress reports, and other relevant documentation.
- Collaboration: Work closely with other professionals, such as social workers, healthcare providers, and legal advisors, to ensure a coordinated approach to client care.
- Compliance: Ensure that all activities comply with legal, regulatory, and organizational policies and standards.
- Experience: Minimum of 1 year of experience in housing advocacy, case management, social services, or a related field.
- Skills:
- Strong interpersonal and communication skills.
- Excellent organizational and time-management abilities.
- Ability to assess and prioritize client needs effectively.
- Proficiency with case management software and documentation practices.
- Problem-solving skills and the ability to work independently as well as part of a team.
Pay Details: $24.00 to $25.00 per hour
