ANCHomeHealthService is hiring: LPN Needed In Philadelphia Area in Philadelphia

We are seeking LPN's to become a part of our team! You will provide routine healthcare to patients at home or in a care facility. Wound care, Diabetic care, Companionship, Medication Administer.

Responsibilities:

• Provide routine individualized care to patient for dependent, non-ambulatory, and dependent for all ADL's.
• Aide must be trained or willing to be trained on: Incontinences care, bed transfers, light housekeeping techniques.
• Monitor and report changes in patient health status.
• Provide personal care, such as bathing and dressing patients.
• Provide specialized healthcare such as administering medication and changing bandages.
• Teach family members ways to care for patient.

Qualifications:

• Must have an LPN certificate, with at least 1 year experience.
• Clean Criminal Background check and Child Abuse clearance.
• Experience in hospice, home health, or other related fields.
• Positive, patient, friendly demeanor
• Ability to build rapport with patients.
• Ability to handle physical workload.
• Excellent written and verbal communication skills.
