We are currently seeking a full time Licensed Practical Nurse (LPN) for our School Based program located in Richmond Community Schools. Hours are Monday - Friday and may fluctuate with clinic needs, 7:30am - 4pm or 8:00 am - 5 pm.
Key Responsibilities of a School Based LPN:
Drug screen, TB test, extensive background check, and fingerprinting are required of all Meridian employees. We are currently seeking a full time Licensed Practical Nurse (LPN) for our School Based program located in Richmond Community Schools. Hours are Monday - Friday and may fluctuate with clinic needs, 7:30am - 4pm or 8:00 am - 5 pm.
Key Responsibilities of a School Based LPN:
- Responds to requests from students/caregivers/clients concerning the effectiveness of medications, side-effects, special precautions or problems with medication and consults with medical staff as appropriate to provide directions on changes/modifications to the prescribed medication regimens.
- Oversee documentation in Medication Administration Record (MAR) and provide corrections as necessary.
- Document as appropriate in the school-based record and the MHS electronic system if a MHS patient visit takes place.
- Phone in prescription orders to pharmacies as directed by medical staff.
- Review lab and test results, reporting concerns and abnormal results to medical staff.
- Obtain medical histories and do physical assessments as directed by medical staff. Including vital signs and other point-of-care testing. Provide health screenings as needed and direct students/clients to appropriate integrated medical care services.
- Administer injectable medications as ordered by medical staff.
- Provide as needed, educational groups, and one-on-one teaching to students/their caregivers and clients concerning their medications, diagnosis and health related issues.
- Complete required clinical documents for review/approval by a member of the medical staff.
- Participate in reviews and modifications of students/clients' services during treatment planning.
- Assist with maintenance of student health records, providing annual screenings, testing and follow up on immunizations through the Indiana Department of Education.
- Function as a liaison with students/caregivers with emergency room, physician offices, pharmacies, and the MHS Provider (if a patient) to clarify medication order, obtain proper prescriptions, labs, testing, and referrals and obtain appointments as needed.
- Complete other documentation to ensure accurate and timely information for client and third-party billing.
- Up to 10% travel for training and coverage at other locations as needed.
- Must be a Licensed Practical Nurse who has successfully completed an Accredited Nursing Program and is currently licensed by the State of Indiana.
- Valid driver's license, proof of auto insurance, reliable transportation, and a driving record meeting Meridian's driving policy
- Two (2) years nursing experience required
