We're looking for someone to join our team as a Community Health Worker who:

The Community Health Worker is a valuable member of the Healthcare for the Homeless team to address the social determinants of homelessness via outreach and enrollment, logistical support, and advocacy/education. The Community Health Worker will conduct outreach to homeless individuals in their surroundings, including encampments, abandoned buildings, parks, meal programs, bus stations, streets, alleys or under bridges to assist in engagement for services.


Essential Functions:


  • Develops and maintains familiarity with client population.
  • Learns and maintains knowledge of the locations and movements of homeless individuals and the social and physical environments, problems and resources used, as well as other patterns or characteristics of the sub-culture.
  • Establishes and maintains regular contact with individuals and other members of the homeless population and familiarizes them with the services provided by CSV to refer into programs.
  • Provides logistical support with enrollment for health insurance programs, housing, and other social services including filling out paperwork, reminders for appointments, transportation to appointments, medication pickup, etc.
  • Develops and maintains familiarity with all relevant public and private agencies and service providers.
  • Provides adequate background, referral information, and coordination necessary for the HCH and other agencies to facilitate linkage to services.
  • Maintains adequate records as to outreach activities, advocacy efforts in the community, and fulfills all other documentation and tracking requirements as required by administration of grants for supportive services.
  • Serves as an advocate and educator for homeless individuals.
  • Provides other services to homeless individuals, as necessary and appropriate.
  • Other duties as assigned.

You'll be successful with the following qualifications:

  • Education: High school diploma or GED equivalent
  • Experience working with homeless population and persons from diverse cultures and income levels.
  • Bilingual proficiency in second language is preferred.
  • Intermediate skill level in the use of a PC and related software applications (MS Office, Access and Excel)
  • Excellent verbal, written communication and listening skills.
  • Strong interpersonal skills with a demonstrated ability to work independently on individual and assigned tasks while exhibiting good judgment.
  • Demonstrated ability to maintain confidentiality.
  • Ability to manage multiple competing priorities and work well in a fast-paced environment.
  • Understanding of basic healthcare principles with the ability to make reasonable and appropriate referrals.
  • Ability to work as a team member.
  • Ability to work some evenings and weekends.
  • Must adhere to Clinica Sierra Vista’s employee health/immunization requirements or provide a valid exemption request for subsequent approval.
