About the role
As a Nurse Advisor at PHM, you'll be the first-person clients engage with clinically as you triage their clinical issues and guide them to the right team at PHM. You'll also support existing clinically complex and high-utilizing clients in navigating healthcare needs assisting them directly or triaging them appropriately. Clients use PHM for a wide range of clinical issues, ranging from simple referrals to helping navigate new cancer diagnosis. As a Nurse Advisor, you will evaluate their needs, explain the services most suitable to their needs, and begin their journey to the best that medicine has to offer.
How You'll Spend Your Days
- Intake & Triage: Lead or participate in clinical intake meetings, triage incoming calls and inquiries on range of issues (urgent, non-urgent) from existing and new clients and arrange for next steps based on the client's needs.
- Manage Health Advisory cases: Oversee a caseload of clients with low to moderate acuity, focusing on care coordination, health advocacy, and navigation of healthcare systems
- Communicate: Voice clients' clinical issues with team leaders and prepare them appropriately to best serve clients.
- Collaborate: Work with internal teams to schedule client appointments, medical records transfer, calendar tracking, and client follow-up communication and participate in client/physician meeting and follow-up on action items
- Coordinate: Select appropriate physicians for clients, oversee coordinator of appointments and care for clients
What you bring to the table:
- An RN license in your home state with at least 5-years of clinical experience.
- Experience triaging patients in an urgent care or emergency room setting, ideally combined with complex clinical experience in a disease specialty.
- You're technologically adept and adapt well to new and existing technologies with ease.
- You're a great team member. You've experience collaborating with and organizing internal & external partners, across multiple departments and time zones.
- You hold yourself and others to the highest standards of ethics and integrity, with a demonstrated capacity for discretion and handling confidential information with care.
- Exceptional communication skills. You communicate clearly, calmly and confidently with patients in acute clinical urgencies both verbally and in writing.

Compensation: The target base salary for this position is $85,000-$100,000 annually.
This base salary is only a part of a total compensation package that also includes: health/dental/vision benefits, 401k with match, PTO and other benefits. Individual pay may vary from the target range as several factors including market forces, experience, location, disparities in market data and other relevant business considerations may all factor into final compensation.
