Job Description
Essential Job Duties: Performs triage activities and prepares patient for physical examination. Updates and revises all patient medical history as it pertains to each specific visit. Explains treatment procedures to patients, if applicable. Assists provider during patient examination as requested by provider. Administers ordered injections as well as assist with minor surgical procedures, if needed. Assists patient with follow up activities to include coordinate scheduling of ancillary services, diagnostic tests, and surgical procedures if applicable. Maintain supplies in and keep exam rooms and back office neat and clean. Attend to phone calls from hospitals, nursing homes, patients, etc. required for continuity of patient care. Serves as a clinical resource to nursing home providers concerning hospice and palliative care practices. Comply with HIPPA, CLIA, OSHA, and office policies and procedures. Demonstrates understanding of hospice and palliative care. Serves as a positive role model to the program and conveys a positive organizational image. Demonstrates timely, accurate, and appropriate documentation. Communicates effectively with the interdisciplinary team to ensure the information is conveyed to all disciplines for continuing care of the patient. Acts as a liaison and advocate for palliative care and hospice patients admitted.

Qualifications
Education: Graduate from an accredited School of Nursing Experience: Registered Nurse license from the Indiana State Board of Nursing.
