Community Health Worker- Healthy Breathing

A Promotor(a)/CHW is a patient advocate/liaison who provides basic, low cost / free health education and resources to communities they are familiar with or have ethnic/cultural/historical ties to. They can help populations that are often disconnected from health systems gain access and learn how to navigate them with confidence and basic health knowledge. An Asthma Promotor(a)/CHW supports clients with asthma in person or via telehealth. They provide asthma education, connect clients to resources and provide necessary mitigative equipment to help with asthma control.
The chief duties of an Asthma Promotor(a)/ CHW include:
- Create connections between special populations and the health care systems
- Provide asthma education in person or virtual
- Conduct visual assessments for asthma triggers in person or virtual
- Participate in annual asthma education courses or workshops
- Care coordination and care transitions for clients
- Assist clients with enrollment in programs and benefits for which they are eligible
- Encourage cultural competence among health care professionals serving special populations
- Advocate for special populations within the health care system and the community at large
- Provide culturally competent health education programs
- Collect health related data
- Maintain records
- Provide healthcare referrals
- Engage in community outreach
- Provide social support and individual counseling
- Basic public speaking skills

Education
Proficient in navigating Los Angeles County’s Health Systems. Written and oral fluency of both English and Spanish is preferred but not required. Have personal experience working in a multicultural setting and at least one year of on-the-job experience is preferred. Computer literacy is necessary for this position. Asthma Promotor(a)/CHW must prioritize a sense of community connectedness as an important asset to the job.
5 years’ experience with working as a community liaison in positions such as, but limited to: Public Health Educator, Health Educator, Peer Counselor, Social Service Coordinator, Social Worker, Social and Human Service Assistant.
Or
Bachelor of Science in, but not limited to: Public Health, Health Innovation, Health Integration, Health Education or Community Health and Development
