N/A JOB SUMMARY: The WISe Youth Partner supports youth and families that have serious emotional disturbance in identifying goals that promote recovery and resiliency using the Wraparound process. This role improves the operations of WBH in support of its stated mission and provides the highest level of customer service to internal and external customers. The WISe Youth Partner connects with youth, parents, and providers in advocating for children with complex behavioral and emotional challenges. Responsibilities include: Assisting with intensive care coordination services for WISe clients and families, including individual, group, family counseling, crisis intervention, and case management services. Facilitating Child and Family Team (CFT) meetings to ensure access, voice, and choice within the wraparound process and to support the family’s connection to the CFT members. Creating the Plan of Care for each family in WISe, ensuring updates after each CFT meeting and completion of action steps assigned during meetings. Completing a CANS screening and assessment on WISe referred children/youth and documenting client progress. Completing all required progress notes according to WBH guidelines and documenting treatment modalities and interventions. Providing and coordinating outreach and crisis intervention services to families in the community. Developing and leading recreational activities for children and families. GENERAL RESPONSIBILITIES: Follow Willapa Behavioral Health & Wellness Service Excellence Standards / Code of Ethics and Policies and Procedures. Adhere to rules of confidentiality. Respect and accommodate a diverse population. Maintain appropriate boundaries with clients and staff. Work on-site or remotely using telehealth technology as determined by management. ESSENTIAL FUNCTIONS: Provide peer counseling and support, encouraging youth to regain hope in and control over their lives and family environment. Assist with developing a collaborative treatment plan. Engage clients and team in treatment following WISe protocol. Complete all documentation using strengths-based language. Collaborate with clinicians and teams to ensure provision of WISe Youth Partner services. JOB REQUIREMENTS: MINIMUM QUALIFICATIONS: High school diploma or equivalent. Must be a self-identified past consumer of mental health services. Must be at least 21 years of age. Agency Affiliated Certification from Washington State. Washington State Certified Peer with High School Diploma or equivalent and experience within 90 days. WISe Training completed within 90 days. Excellent verbal and written communication skills, computer proficiency. Undergo and pass a criminal background check. Meet agency’s drug-free workplace policy. Excellent customer service skills. Able to sit, stand, and move for long hours and lift up to 25 pounds. A valid driver’s license and reliable vehicle. Able to drive in various weather conditions. Flexible to fill-in shifts as needed. Meet current Washington health care vaccination requirements. PREFERRED QUALIFICATIONS: Experience working as a peer in behavioral health or substance abuse program.
