Certification Details BLS State licensure or compact license Additional Details Covid vaccine required, provide vaccination card with submission. Declination will not be accepted for any reason, only submit candidates who are fully vaccinated or have had the first shot with the second already scheduled. Documentation of 2 MMRs at least 28 days apart and/or positive laboratory titer Fit Test required. Hepatitis B documentation needed (series of 3) and/or positive laboratory titer or signed declination. Influenza vaccine status required, with exemptions reviewed for religious or medical reasons only. TB Test: 2 Negative TB skin tests or Quantiferon Gold required. Documentation of 2 doses of varicella 4 weeks apart and/or positive laboratory titer Proof of having received Tdap vaccine required. Physical Capacity/Health Statement written by authorized individual.
