* JOB SUMMARY

* Draws blood from patients in hospital, or similar facility, for analysis or other medical purposes

* DUTIES AND ESSENTIAL FUNCTIONS

* Overall performance rating of below indicated essential job duties:

* Assembles equipment, such as tourniquet, needles, disposable containers for needles, blood collection devices, gauze, cotton and alcohol, on work tray in accordance to requirements for specified tests of procedures

* Verifies or records identity of patient and converses with patient to allay fear of procedure

* Correctly labels blood collected from patient listing patients name, medical record number, physician, date and time collected, and initials

* Correctly verifies in the computer the blood was collected

* Deliver the specimen collected to the correct department in the laboratory

* Regularly checks the collected and uncollected lists

* Attends mandatory training programs

* EDUCATION/LICENSE/ CERTIFICATIONS/OTHER REQUIREMENTS

* Minimum requirement:

* High school diploma or Associate's degree (A.A.) or equivalent from two year college or technical school; or completion of a certified phlebotomy course or one year related experience and/or training

* National Phlebotomist certification through accredited program

* Preferred qualifications:

* WORKING ENVIRONMENT

* This job operates in a professional medical laboratory environment

* Regularly works near moving mechanical parts

* Constantly exposed to blood and/or body fluids and infection.

* Occasionally exposed to toxic or caustic chemicals

* Occasionally exposed to risk of radiation

* The noise level in the work environment is usually moderate.

* PHYSICAL DEMANDS

* Occasionally required to maintain a stationary position (standing or sitting)

* Frequently required to walk and move about

* Frequently required to operate a computer and other equipment

* Frequently required to use fine motor skills to handle and operate instruments/devices

* Frequently required to position self in order to meet patient needs

* Regularly required to communicate with patients and other care team requiring talking and hearing

* Frequently required to lift and/or move up to 25 pounds.

* Occasionally required to life and/or move up to 50 pounds.

* Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, and depth perception.
