Primary Responsibilities:

  • Answers and distributes phone calls for designated team
  • Communicates with internal and external customers to address any needs or concerns
  • Provides communication in a non-judgmental, non-discriminatory manner that is sensitive to the patient/family diversity, preserving their autonomy, dignity and rights
  • Reports patient condition and needed follow-up to appropriate personnel in a timely manner
  • Tracks Recertification Periods and alert Medical Directors, Patient Care Mangers, and RN Case Managers of upcoming recertification
  • Print daily medication orders and fax to the Physician/Nurse Practitioner /Pharmacy
  • Prepares an Agenda and CM folders for Weekly IDG and team meetings with all updates
  • Print medication profiles for new admission and recertification patients for IDG
  • Reports updates and changes to afterhours and team as necessary
  • Maintains and update the current census reports
  • Verify accuracy of data entry from completed admissions
  • Ensure verbal certification is obtained within 48 hours of admission
  • Obtain physician signatures for initial CTI's and Plan of Care timely
  • Obtain additional medical records from physician offices and hospitals to support terminal illness, and any other records requested by the team
  • File and keep medical records current
  • Participates in special projects and performs other duties as assigned

Knowledge and Skills Requirements:

  • Ability to establish and maintain effective working relationships with staff, patient/family members, the public, and representatives of other agencies and organizations
  • Ability to communicate effectively and professionally
  • Strong customer service skills and helpful attitude
  • Organized, able to manage multiple tasks at a time, and prioritize tasks
  • Ability to maintain records and prepare related reports

Experience and Educational Requirements:

  • Medical Assistant, or 3 years of medical secretarial experience preferred
  • Must have a strong knowledge of medical terminology.

Salary Range:

$21.00 To $25.00 Per Hour
