We are seeking a skilled Home Health Registered Nurse (RN) for a 13-week travel assignment in the dynamic city of Indianapolis, Indiana. Provide compassionate care to patients in their homes, ensuring their safety, health, and recovery through personalized care plans.

• Location: Indianapolis, Indiana
• Schedule: Flexible (may vary based on patient needs)
• Assignment Duration: 13 weeks
• Start Date: ASAP

• Conduct comprehensive assessments of patients in their home environments.
• Develop, monitor, and revise individualized care plans.
• Administer medications, provide wound care, and perform other skilled nursing procedures.
• Educate patients and families on care plans and self-management.
• Collaborate with interdisciplinary teams to coordinate patient care effectively.

• Active Indiana RN license (or compact license).
• Minimum 1 year of recent home health experience.
• Proficiency with OASIS documentation.
• Current BLS certification (Aman Heart Association preferred).
• Excellent communication and organizational skills.
