About the Role:

We are seeking a highly skilled and motivated MDSRN to join our team at Seneca House. As an MDSRN, you will be responsible for ensuring the highest level of care for our patients. You will work closely with our interdisciplinary team to develop and implement care plans, monitor patient progress, and provide ongoing support to patients and their families. Your attention to detail and ability to work collaboratively will be essential to the success of this role.

Minimum Qualifications:

• Active MDSRN license
• Minimum of 2 years of experience in a similar role
• Strong communication and interpersonal skills
• Ability to work collaboratively with an interdisciplinary team
• Excellent attention to detail

Preferred Qualifications:

• Bachelor's degree in nursing
• Experience working with geriatric patients
• Certification in MDS assessment
• Experience with electronic medical records

Responsibilities:

• Develop and implement care plans for patients
• Monitor patient progress and adjust care plans as needed
• Provide ongoing support to patients and their families
• Collaborate with interdisciplinary team to ensure the highest level of care
• Maintain accurate and up-to-date patient records
