Salary: $82,000 - $92,000 (Depending on Experience)
Position Type: On-Site/Hybrid
As a member of our Cosán Group team, the RN Case Manager will coordinate and oversee care for assigned patient population to ensure access to high-quality and cost-effective healthcare services. This role involves creating individualized care plans, collaborating with multidisciplinary teams, and acting as a liaison between patients, healthcare providers, and community resources. The Case Manager will ensure that patients receive the services they need while promoting their overall well-being and independence.

Key Responsibilities:
- Care Coordination and Case Management:
- Assess patients' medical, psychological, and social needs to develop comprehensive care plans.
- Collaborate with healthcare providers, specialists, and community resources to facilitate coordinated care.
- Monitor patient progress and adjust care plans as needed.
- Patient Advocacy and Support:
- Serve as the primary point of contact for patients and their families to address concerns and answer questions.
- Advocate for patients’ rights and access to necessary services and resources.
- Resource Management:
- Identify and connect patients with appropriate healthcare and community services.
- Assist with understanding and accessing available insurance benefits and support options.
- Documentation and Compliance:
- Maintain accurate, up-to-date case records and documentation in compliance with organizational policies and regulatory requirements.
- Ensure adherence to HIPAA and other privacy standards.
- Education and Outreach:
- Provide patients and families with education about their condition, treatment plans, and available resources.
- Conduct outreach to build relationships with community partners and resource networks.
- Education and Experience:
- Associate's degree in nursing, bachelor's degree in nursing preferred.
- Previous experience in case management, healthcare coordination, or a similar role (2–4 years preferred).
- Certifications (Required/Preferred):
- Active compact, multistate license as a Registered Nurse (RN) (required)
- Certified Case Manager (CCM) credential (preferred).
- Skills and Competencies:
- Strong interpersonal and communication skills.
- Ability to manage multiple cases effectively with attention to detail and prioritization.
- Knowledge of healthcare systems, insurance processes, and community resources.
- Proficiency in electronic health record (EHR) systems and case management software.
- Hybrid office-based
- May involve occasional weekend or evening hours based on patient needs.
