Licensed Clinical Social Worker (LCSW) – Hamburg, NY

Harmonia Collaborative Care is seeking a compassionate Licensed Clinical Social Worker (LCSW) to join our dynamic mental health team. We are dedicated to improving lives through patient-centered care in a supportive and collaborative work environment.

Key Responsibilities

• Conduct mental health assessments and develop treatment plans
• Provide evidence-based therapy (CBT, DBT, MI, etc.)
• Collaborate with a multidisciplinary team to ensure holistic care
• Maintain accurate clinical documentation

Requirements

• Master’s degree in Social Work and valid LCSW license in NY
• Minimum 2 years post-licensure clinical experience
• Strong interpersonal and teamwork skills
