GENERAL SUMMARY:
The Oncology Social Worker, in collaboration with the oncologists and midlevel providers, serves as the primary provider of psychosocial care to cancer patients and their families seen in the outpatient clinics of the Bayhealth Cancer Center. The Oncology Social Worker will facilitate adjustment of patient and families to a cancer diagnosis, treatment and recovery by promoting coping skills, self-advocacy, sharing information and resource referrals.
ESSENTIAL FUNCTIONS & ACCOUNTABILITIES:
1. Psychosocial Assessment of Cancer Patients: To provide psychosocial screening for the patient, family and/or caregiver at pivotal medical visits across the cancer care continuum: prevention, diagnosis, treatment, survivorship, palliative care, end of life care and bereavement. Demonstrate the ability to recognize, treat, monitor and refer to appropriate resources when patient experiences distress throughout the disease trajectory - from diagnosis through treatment into survivorship or palliative and/or end-of-life care.
2. Multidisciplinary Team Based Care: Works with the patient, patient's family and other interdisciplinary team members to provide assessment and treatment plan, including client needs; mental status; support systems; financial, community, and government resources; options for short-term counseling; development of goals and treatment plans; crisis counseling; prevention activities and coaching skills. Active participant in multidisciplinary conferences.
3. Cancer Patient Navigation: To assess and remove barriers to care across the continuum, coordinate with appropriate resources. To provide counseling and education regarding cancer diagnosis and treatment with the patient, family and/or caregiver, facilitating referrals to appropriate disciplines and community resources.
4. Patient and Family Referrals: Conducts on-site individual counseling, facilitates educational groups, and offers support groups and/or supportive services during treatment period. Makes referrals to community mental health services for ongoing therapeutic needs. Make referrals to appropriate community agencies, equipment, services as needed. Collaborate with inpatient social work teams and clinical resource management department when indicated to support patient care continuum.
5. Documentation: Completes documentation and assessments accurately for each patient encounter.
6. Community Outreach: Serves as a resource for community organizations to promote and educate on psychosocial services for cancer patients.
7. All other duties as assigned within the scope and range of job responsibilities

Required Education, Credential(s) and Experience:
- Education: Master Degree in Social Work is required
- Credential: Licensed Clinical Social Worker is required
- Experience: Two (2) years' experience in a hospital or Medical Center Social Work setting is required
- Education:
- Credential: Basic Life Support; Oncology Social Work Certification
- Experience: Experience with Acute Care discharge planning Experience with outpatient discharge planning
