In so doing, the pharmacist must have current, specialized drug knowledge and demonstrate judgment, communication skills, self-direction and motivation to affect positive interventions in patient outcomes beyond those exhibited by those in a Clinical Pharmacist position. Participate in educational programs, as necessary, to provide basic and advanced services of the Pharmacy Department. Additionally, perform in the Clinical Pharmacist role with its accountabilities and job activities pertaining to drug distribution, supervision and non-dispensing functions. The Critical Care Pharmacist is responsible for delivering comprehensive pharmaceutical care to critically ill patients within the intensive care unit (ICU). This role involves active participation in multidisciplinary rounds, collaboration with healthcare providers, and oversight of medication management processes. The pharmacist will also lead clinical initiatives, provide education, and ensure adherence to best practices in critical care pharmacotherapy. Key Responsibilities: Clinical Rounding: Actively participate in daily multidisciplinary rounds with physicians, nurses, and other healthcare professionals to develop and implement optimal medication therapy plans for critically ill patients. Provide expert pharmacotherapeutic recommendations tailored to individual patient needs, ensuring safe and effective medication use. Order Verification and Medication Management: Review and verify medication orders in the ICU, ensuring accuracy, appropriateness, and compliance with established protocols. Monitor patient responses to therapy, adjusting medication regimens as necessary based on clinical status and laboratory results. Education and Training: Serve as a resource and mentor for pharmacy staff, residents, and students, providing education on critical care pharmacotherapy. Conduct training sessions for ICU nursing and medical staff on medication safety, new therapies, and best practices. Policy Development and Quality Improvement: Lead the development and implementation of medication-related policies, protocols, and guidelines specific to critical care. Participate in quality improvement initiatives aimed at enhancing patient outcomes and medication safety within the ICU.
