• Monday-Friday: 8:00am-close (close at 4:30pm but can be delayed by walk in injuries)

• Average Number of Patients per Day: 8-10
• Number of Exam Rooms: 5
• Number of Physicians in Practice: 1

Requirements

• Board Certified
• Active VA License
• BLS/ACLS Certified
• DOT Certification
• Provider must be willing to work at both sites nearby each other
• Required Duties: Pre-Employment Physicals, Return-to Duty Testing, DOT Physicals, workers compensation injury care
• Physician Assistant-Certified (PA-C)
