Who We Are
Sabathani Community Center (SCC) is a 58-year-old African American institution and nonprofit serving a multi-cultural community of 50,000 community members in South Minneapolis. SCC was founded in 1966 by a handful of concerned community members seeking to address social and economic disparities in underserved communities in the areas of youth programming, housing, health and education. Wages & Benefits
$25 - $27 per hour ; generous paid time off (PTO), plus 11 paid holidays and two floating holidays; competitive health, dental, and vision insurance; Health Savings Account (HSA), and options for Flexible Spending Accounts (FSA). There is employer paid basic life insurance with the option for additional coverage paid by the employee. The employer pays for Accidental Death and Dismemberment (AD&D) and short- and long-term disability insurance. SCC sponsors a retirement plan with 4% employer match and immediate vesting.
A Typical Day in the Life...
A typical day as Sabathani's Family Liaison Specialist provides an opportunity to implement programs and activities that strengthen and support African American families, and will include the following:
Assessing participants' readiness for services and determining their ability and willingness to participate.
Creating case plans with short, medium, and long-term goals and steps to achieve them.
Assisting in developing supportive parenting plans.
Handling enrollment applications and intake processes.
Preparing reports on outcomes, successes, and challenges.
Building and maintaining relationships with community resources.
Providing referrals to agencies and service providers based on participants' goals.
Maintaining participant records and data in Salesforce.
Advocating for participants to reduce barriers to needed services.
Following up to ensure participants are receiving services in a timely manner.
Networking with partner agencies, coalitions, and community members.
Sharing information about resources and services in the community.
Working with the supervisor to design and improve program strategies.
Conducting needs assessments to identify service gaps.
Who You Are
You have a bachelor's degree in a Human Services, Social Services, or a related field or demonstrably equivalent knowledge gained through alternate courses of study and life experience.
You have a minimum of five years of work experience in Human Services or Social Services.
You bring experience in non-profit management, case management, and/or program development.
You preferably have experience working within the African American community.
You communicate clearly and effectively through both oral and written means.
You present a friendly and positive demeanor to veterans, their families, staff, supportive services personnel, funders, and the public, and you can make effective oral presentations.
You stay calm and maintain self-control during difficult situations and emergencies.
You adapt well to change and respond professionally in all situations.
You have a working knowledge of Outlook, Word, and Excel, with PowerPoint and Access as a plus.
You think analytically and evaluate the impact of your case management recommendations.
You set work priorities effectively and develop solutions to work-related challenges.
You maintain clear boundaries in your role.
You have strong math skills and can assist veterans in creating a budget.
You are knowledgeable about case management services and community resources.
You can maintain current CPR, AED, and BBP certifications. (For new hires, training will be provided during onboarding.)
You have a valid driver's license, access to reliable transportation, and the ability to meet Sabathani's driving record requirements. Please note that if the license was
