Discharge Planner (Home Health Transitions) , you will coordinate post-hospital care by arranging in-home services, equipment, and follow-up appointments. Your thorough planning and communication ensure seniors experience a seamless discharge, reducing readmissions and improving outcomes. Key Responsibilities:
Work with hospital staff, insurers, and families to create comprehensive discharge plans. Arrange home health services, medical equipment, and medications. Schedule follow-up appointments and coordinate transportation as needed. Educate patients and caregivers on discharge instructions and care plans. Qualifications:
