is a California-licensed healthcare company that provides quality medical care to our home-bound patients. Overview: We are seeking FNPs to visit patients' homes, apartments, or nursing facilities, in order to both establish new patients, as well as provide follow-up care. This is a per diem position. Our team will schedule you with Medicare patients (referred from home health agencies) per your availability, in your local area. Nurse Practitioners will be able to access patient charts, input patient notes and diagnosis codes into our EHR system (Office Ally), as well as order labs and/or prescriptions for patients, and narcotics (provided practitioner obtains a DEA license), via the online pharmacy. This will be a collaborative agreement between the practitioner and our supervising Physician. Nurse Practitioners will act as PCPs for ACT Medical Group.

Requirements and Qualifications:

Master's Degree. Board certification and license (RN, FNP). Active DEA license or ability to obtain. NPI number and furnishing license. An outstanding observation and analytical skill to diagnose and treat the patient effectively. Excellent verbal, written, and interpersonal skills. Fully bilingual in both English and Spanish is a plus. Compassionate and skilled Nurse Practitioner. Eager to provide top-notch care to patients at home. Professional seeking flexible work schedule. Available day(s) Monday - Saturday. Job Summary:

Provide clinical care and consultation. Prescribe necessary treatment within the scope of practice and assess the patient's clinical problems and healthcare needs. Prescribe medications in compliance with established medical protocols and in compliance with government agencies and guidelines. Educate patients on preventive care and health safety issues. Evaluate and treat patients in the skilled nursing facility setting or post-discharge at the patient's residential facility or private residence. Perform annual wellness exams for senior Medicare patients at a residential facility or clinic setting. Cover for other primary care physicians when necessary. Evaluate and treat patients independently. Obtain the patient's medical history and documentation in EMR. Create a progress note for each encounter with each patient you evaluated. A progress note will include Subjective, Objective, Assessment, and Plan sections, and must be completed after each examination. Promote a friendly, warm, and exceptional customer experience. Secures patient information and maintains patient confidence by completing and safeguarding medical records; keeping patient information confidential by abiding by all HIPAA rules and regulations. Prescribe pharmaceuticals, medications, and treatment regimens as appropriate to assessed medical conditions. Any additional duties as assigned by the Medical Director, MDs, and Director of Operations.
