The Integrated Care Center Clinic is a federally qualified health center that provides primary care, behavioral health, addiction treatment, psychiatry, dental care, and more. We provide care to those most in need, including those who are suffering from homelessness, mental illness, substance use, and chronic medical conditions. We provide team-based care in an integrated care setting. Position Overview: The On-Call Medical Referrals Coordinator (MRC) will be responsible for all coordination of care for clinic patients. The MRC works to ensure the effective management of all incoming specialist referrals. This position will perform a wide variety of tasks associated with referral tracking to be compliant with Patient Centered Medical Home (PCMH) requirements to ensure quality patient care and safety. Key Responsibilities: Administrative Responsibilities:
Ensure that patient’s primary care chart is up to date with information on specialty consults, hospitalizations, ER visits, and chronic disease management related to their health. Entry of patient clinical and non-clinical information into assigned electronic registry is a key responsibility of this position (e.g., e-Clinicalworks, CareWare, Aries, +Care, etc.). Maintain ongoing tracking (SFGH alerts) to the Primary care provider team and appropriate documentation on referrals to promote team awareness and ensure patient safety. Advocacy Responsibilities:
Effectively assist patients in problem-solving potential issues related to the healthcare system, financial or social barriers (e.g., request interpreters as appropriate, transportation services, or prescription assistance). Act as the system navigator and point of contact for patients and providers. May assume the advocate role on the patient’s behalf with the carrier to ensure approval of necessary services for the patient in a timely fashion. Assist patients with signing up for medical coverage. Qualifications: Education and Experience Required: High school diploma combined with medical/clinical experience. Experience working as a patient advocate. Baccalaureate degree in a related field. Must be able to complete a background check. Knowledge Required: Culturally competent and able to work with a diverse population. Strong proficiency with Microsoft Office applications, specifically Word, Outlook, and internet applications. Experience working successfully with issues of substance abuse, criminal background, and other potential barriers to economic self-sufficiency. Organized and able to manage competing priorities. Resourcefulness in problem-solving. Covid-19 vaccine and booster are required for this position. Exemptions are not accepted.
