Saint Francis Hospital and Mercy Fitzgerald Hospital, members of Trinity Health Mid-Atlantic, is seeking experienced Director of Care Management South to join the Case Management team! Schedule: Full-time, 40hrs/wk, Monday-Friday day shift. The Director of Care Management is responsible for development, implementation, direction, coordination and oversight of the Care Management Department for our South region that includes both
Saint Francis Hospital in Wilmington, DE and Mercy Fitzgerald Hospital in Darby, PA . The role requires collaborative, proactive and patient centered relationships to achieve consensus and to facilitate and maximize outcomes. This includes collaboration with community, local and state resources. The position requires specialized knowledge, training and experience. Also requires current knowledge and understanding of laws, legal and professional requirements, privacy, confidentiality and other principles and rules. Analyzes trends in healthcare and their effectiveness in the application of such to the CM department. Participation in data collection and use of tools to accomplish these goals. Provides the framework in which outcome goals can be achieved through integration of the utilization, clinical, and discharge management functions of the CM department. Responsibilities include:
Maintains collaborative relationship with Medical Staff and facilitates education on compliance related utilization management issues. Facilitates the Care Management Council and assures adherence to the required components in accordance with the CoPs. Ensures system adherence of all Federal, State, and Local clinical and regulatory guidelines related to utilization. Ensures oversight of critical processes related to compliance of inpatient, observation, ambulatory claims, and appropriate level of care. Develops collaborative relationships with physicians, nurses and other patient care support colleagues. Develops collaborative relationships with payers, other providers and referral sources to facilitate appropriate utilization of related clinical services and innovative care management initiatives. Based on evidence and care management opportunities, collaborates with service line administrators to adjust the common care strategy to accommodate service line goals and objectives. Minimum Qualifications:
Licensed RN in the State of Delaware and Pennsylvania or Masters level Social Worker; American Case Management Association (ACMA) accreditation also recommended. BSN -or- MSW degree. Ten years experience in Case Management or in Medical/Surgical Acute Care setting. Care Management, Utilization, Social Services, Infection Control and Performance Improvement experience required.
