Job Duties

As a key member of our team, you will be accountable for the assessment, coordination, delivery, and evaluation of nursing care, including direct patient care, patient/family education, and transitions of care in an acute patient setting. You will deliver holistic and individualized care to all patients in an assigned area, developing and implementing a plan of care that incorporates psychological, sociocultural, spiritual, economic, and lifestyle factors.

Job AccountabilitiesAssessment and Planning of Patient Care
  • Conduct patient and family assessments encompassing physiological, spiritual, and emotional areas.
  • Collaborate and communicate with physicians and other healthcare team members to interpret, adjust, and coordinate daily patient care plans.
  • Partner with peers, management, and other healthcare providers to streamline patient workflow and improve outcomes.
  • Communicate patients' plan of care to co-workers and during shift changes.
Provision of Patient Care
  • Provide competent, compassionate, and cost-effective patient care in accordance with policies and standards of care.
  • Maintain a safe, comfortable, and therapeutic environment for patients and their families.
  • Delegate tasks appropriately to team members based on legal scopes of practice and individual capability.
  • Communicate treatments and activities to patients and families in a collaborative manner, ensuring understanding and integrating educational needs.
  • Maintain mindfulness in delivering individualized care and communication with a safety focus.
  • Uphold confidentiality of all patients' protected health information (PHI).
  • Stay current in advancements, trends, and best practices related to acute care and specialty areas.
Evaluation of Patient Care
  • Monitor patient care interventions, assess therapies, evaluate outcomes, and intervene as warranted.
  • Conduct ongoing education for patients and their families to ensure optimal care from admission through transitions.
  • Reassess, evaluate, and document each patient's response to the plan of care and adjust as needed.
  • Report safety or compliance concerns and participate in safety events to prevent future occurrences.
  • Develop, implement, and document formal teaching and transition planning that includes patient/family involvement.
  • Verify and validate competency of newly hired and/or student staff as appropriate.
  • Serve as a clinical resource and mentor to newly hired staff, assisting with orientation as requested.
