Job Description:
Contract- 13 weeks with higher chance extension or Perm Shift: Days - 8.00 am to 4.00pm
Location- 200 Mt. Airy Road, Basking ridge, NJ 07920 Job Duties: Attends monthly staff meeting:Participates in PI meetings and activities/committees as requested. Accepts other assignments as requested.Assumes responsibility for case management: Prioritizes schedule to accommodate patient's needs. Documents case management activities. Demonstrates knowledge of pharmacology and safe medication management. Routinely reconciles medications. Maintain current medication orders.Participates in Performance Improvement Program: Reviews health records and gathers data to support PI activities. Attends UR meetings as requested. Provides orientation/ mentorship to new employees. Cross trains to learn new skills and demonstrates a willingness to teach otherAssists with growing agency business: Provides orientation to new employees. Accepts other assignments as requested. Cross trains to learn new skills.
Schedule notes: Days per week: 5 Shift: 8a-4p On call or weekends required? 1 weekend per month and possibly an additional day on a weekend Float: Yes, from county to county (We cover Morris, Somerset, Union, and Essex)
