We are seeking a Licensed Clinical Social Worker (LCSW) to provide high-quality psychosocial support in the Inpatient and Emergency Room (ER) settings . This role involves performing biopsychosocial assessments, crisis intervention, and counseling services, as well as advocating for patients and families. The position includes working with a diverse patient population of all ages, collaborating with the multidisciplinary healthcare team to ensure smooth transitions of care and discharge planning. The ideal candidate will have inpatient and ER experience. Position Highlights: Schedule: Monday – Friday with rotating weekends, 8:00 AM – 4:30 PM Setting: Inpatient and ER departments Required Experience: Inpatient and ER experience preferred Open to First-Time Travelers: Yes Job Responsibilities: Perform biopsychosocial assessments to understand patients’ social, emotional, and medical needs and develop appropriate care plans. Provide crisis intervention , individual and family counseling , and short-term therapy as needed to support patients and families. Coordinate discharge planning and facilitate transitions of care , including linking patients and families with appropriate community resources and services. Advocate for patients and families by ensuring they have access to necessary care, services, and resources, promoting self-determination, and addressing psychosocial barriers. Work collaboratively with interdisciplinary teams to provide comprehensive care and consult on psychosocial aspects of patient care. Document all patient interactions in the medical record, ensuring accuracy, clarity, and confidentiality. Provide psycho-education and facilitate support groups for patients, families, and hospital staff. Serve as a liaison to community programs and resources, ensuring continuity of care after discharge. Maintain professional relationships with hospital departments and community agencies, contributing to patient care coordination and process improvement initiatives.
