This position is responsible for coordination of a high level of service and quality of care to individuals requesting outpatient service for the Lisa Dean Moseley Foundation Institute for Cancer and Blood Disorders (LDMFI-CBD) and sub-specialties where patients of the LDMFI-CBD are treated. The Inpatient/Outpatient Transplant and Cellular Therapy (TCT) Nurse Navigator will serve as an advocate for complex patients, their families, and caregivers, to overcome health care system barriers.

The primary functions include:

Improve patient's access to care by assuring efficient and effective coordination of patient care between admissions, visits, including scans, complex scheduling, and sub-specialty interface. Triage newly diagnosed patient and family with complex needs in the inpatient and outpatient setting and provide procedure related information during the scheduling process. Collaborate with the physicians, members of the healthcare team, and the patient/family to maintain a plan to deliver quality healthcare in the inpatient and outpatient setting, and in the home environment, focusing on educational and resource needs. Provide guidance and education for the highly complex patient and family to ensure the patient receives the necessary care, throughout the continuum of care. Coordination and delegation for prior authorizations and peer to peer for highly complex patients. Collaborate with the LDMFI-CBD specialty nurses for care coordination and cross coverage as needed. Evaluate the patient/family response to care on an ongoing basis.

Requirements

BSN from accredited program

Current, unrestricted DE Registered Nurse License required (subject to change based on patient base).

Specialty Certification: CPON/CPHON certification preferred.

Minimum 5 years of pediatric hematology/oncology nursing experience required
