Assist with the development and implementation of individualized care plans. Collaborate with other healthcare professionals to ensure comprehensive care for residents. Maintain accurate patient records, ensuring proper documentation of care provided. Provide support and guidance to nursing assistants and other support staff. Previous experience as an LPN, preferably in long-term care or skilled nursing settings. Strong clinical skills and the ability to assess patient needs and respond accordingly.
