Intake Nurse

Job Description: A healthcare network in New York City is looking to add licensed Registered Nurse (RN) to their intake team in Brooklyn. In this role, the intake nurse (RN) will be responsible for conducting assessments to admit patients to their hospice program. About the Opportunity:Specialty: HospicePatient Population: Adults / GeriatricsSchedule: Monday to FridayHours: Standard business Responsibilities:The Admissions Nurse (RN) will:Perform physical and psychosocial assessments of potential hospice patients in order to develop individualized plans of care. Assessments take place in both private homes and facilities. Admit eligible patients into the hospice program following appropriate standards of practice established by the National Hospice and Palliative Care Organization (NHPCO).Perform other duties, as needed Qualifications:2 years in med-surg, community health, oncology or hospice. Graduate of accredited School of NursingNYS Registered Nurse (RN) license Desired Skills:BSNPrevious hospice experiencePrevious intake or admissions experience.
