Job Summary and Qualifications The Mental Health Professional Lead facilitates high quality, patient-centered care by providing and ensuring optimum psychiatric treatment services to patients and their families. The Mental Health Professional Lead performs assessments, psycho-education, process groups, treatment planning, discharge planning and therapeutic counseling support through evidenced based interventions in individual, family, and group therapy. The Mental Health Professional Lead provides leadership and coaching and serves as a resource for the care team.

What will you do in this role:

Demonstrates advanced clinical skills and ability to plan and facilitate individual, group and family psychoeducational and process groups.

Develops treatment plans, attends and leads treatment planning meetings and reviews on an ongoing basis. Ensures timeliness and thoroughness of treatment planning documentation and attendance.

Assesses environment and patient’s safety status and reports/responds to actual and potential safety issues or changes in the patient's condition.

Identifies risk factors, screening criteria and interventions for patients with special precautions (e.g., suicide, homicide, falls, elopement).

Completes advanced assessments, group notes and all other documentation promptly and in compliance with requirements, as appropriate to scope.

Collaborates with the multidisciplinary team to assess and coordinate patient’s discharge planning needs and readiness for discharge. Provides guidance on complex issues.

Conducts post-discharge follow-up as indicated.

Evaluates re-admissions for gaps in discharge planning processes. Formulates a plan and implements new strategies.

Develops and maintains a community resource guide including access to public assistance/alternatives.

Coordinates and participates in Community Education, Activities and Support Groups, as appropriate to scope.

Assumes responsibility for ensuring a safe, structured milieu with evidence-based treatment modalities/resources.

Identifies cultural considerations, gender identity and preferred language for patients and family.

Demonstrates appropriate application of age/developmental stage specific standards, policies and procedures, and guidelines in assessing and treating patients to ensure patient safety.

Maintains active and timely communication with patients, families, multidisciplinary team and continuum of care providers. Addresses any communication-related issues as appropriate.

Maintains professionalism and uses good communication skills in interacting with peers, staff, patients, families and in resolving conflicts.

Demonstrates understanding of and ability to maintain clear professional boundaries with peers, patients and families.

Provides education and guidance regarding state commitment processes, abuse reporting requirements, duty to warn and guardianship processes.

Demonstrates knowledge and role in hospital codes.

Serves as an educational resource to patients, families, interdisciplinary team members, payer representatives, and other parties as appropriate regarding behavioral services.

Oversees staff provision of care and services, ensuring the use of accepted standards of care.

Assists in interviewing and selecting qualified personnel as required to meet departmental objectives.

Reviews, coaches, and ensures the training of new staff/interns as assigned within approved practice and program guidelines.

Provides clinical supervision of others towards advanced licensure, as appropriate.

Provides initial and ongoing performance review/development of assigned personnel including development opportunities. Utilizes appropriate and regular oversight when delegating competency assessment activities.

Assumes responsibility for appropriate disciplinary action of staff members in collaboration with HR.

Develops department goals/objectives and clinical programming.

Develops Process and Quality Improvement initiatives appropriate to department.

Performs utilization review as assigned.

Assists with Process and Quality Improvement initiatives.

Assumes accountability for departmental operational budget, inclusive of staffing productivity, scheduling, billing and supply management, and utilizing management, as applicable.

Maintains data integrity through auditing to ensure accuracy.

Performs other duties as assigned.

What qualifications you will need:

Masters in Social Work (MSW) required

3+ years experience in mental health or related field required

1+ years experience in leadership preferred

Currently licensed as one of the following professionals in the state(s) of practice, according to law and regulation

REQUIRED:

Licensed Clinical Social Worker (LCWS)

Licensed Professional Counselor (LPC)*

Licensed Marriage and Family Therapist (LMFT)*

Licensed Mental Health Counselor (LMHC)*

Licensed Independent Social Worker (LISW, LICSW)*

Must be authorized by the state to provide outpatient psychiatry treatment services to Medicare beneficiaries.
